low byte = file attributes (see #01401 at INT 21/AH=3Ch)
high byte = 00h normal create, 01h create new file

Return:
CF set on error
AX = DOS error code (see #01680 at INT 21/AH=59h/BX=0000h)
CF clear if successful
SFT filled (except handle count, which DOS manages itself)
STACK unchanged
